Applications are to be sent in the prescribed form. For obtaining an application form, please write to The Director General on the following address along with a fee of Rs.500/- by Demand Draft in favour of National Safety Council payable at Mumbai.

Information furnished in the application form should be approved in the Safety Committee or other Joint Management- Workers Committee.
The application form should be signed by both the representatives of the Management and Workers.
